Under-Chin Fat Reduction

Reduces fullness beneath the chin (submental fat) using an injectable or a device-based approach, helping to sharpen the profile and jawline.

Under-chin fat reduction in Vancouver

Understanding Under-Chin Fat Reduction.

Fullness under the chin is stubborn in a way that feels unfair. It can sit there through weight loss, through exercise, and through a family history you had no say in.

There are two quite different ways to reduce it — an injectable medication, and a cooling-based device. They are not variations on a theme. They work differently, feel different, recover differently, and carry unrelated risks.

Before either one is worth considering, one question has to be answered: is the fullness actually fat? Loose skin and prominent neck muscle bands can look similar in the mirror and respond to neither treatment.

That is what an assessment is for.

Two routes, two different experiences. This is the comparison patients most often want laid side by side.

 Injectable (deoxycholic acid)Cooling device (cryolipolysis)
How it reduces fatA medication injected into the fat disrupts fat-cell membranes; the body clears the cells over following weeksAn applicator cools the area so fat cells are affected and gradually cleared, while skin is comparatively spared
What the session feels likeA grid of small injections; stinging or burning during and afterSuction and intense cold, then numbness; no needles
Visible recoverySwelling that can be pronounced and socially noticeable, plus bruising and firmnessRedness, mild swelling, tenderness and numbness; usually less conspicuous
Risks that are specific to itTemporary uneven smile from nerve irritation, difficulty swallowing, injection-site ulcerationParadoxical adipose hyperplasia, prolonged numbness, late-onset pain
Who performs itA physician — Dr. Gidon Frame, Dr. Sarina Govindasamy or Dr. Teri Fisher — because it is a needle-based procedureOur experienced laser technicians and estheticians, working directly under the guidance of our physicians
Neither one treatsLoose neck skin, vertical neck muscle bands, or overall body weight

Because the risk profiles do not overlap, choosing between these is a genuine medical decision rather than a preference. Both safety sections below are worth reading before you decide.

Treatment options and technologies

Reducing the fat pad beneath the chin can be approached in more than one clinically recognised way, and neither is always the appropriate one — where the fullness is loose skin or platysmal banding rather than fat, neither will help.

The options available here, selected according to clinical findings:

  • Belkyra — considered for moderate to severe submental fullness in adults where the fat pad can be mapped and treated precisely, and where a period of conspicuous swelling is acceptable; it is authorised by Health Canada for this indication and is placed by a physician
  • CoolSculpting — considered where the pocket can be drawn into the applicator and where minimal visible recovery is a priority; no needles and no medication are involved

The two carry different risk profiles rather than being variations on one treatment, so choosing between them is a medical decision. Both safety sections below are worth reading in full.

The selection is a clinical decision, not one you are expected to make. The area is examined by hand to separate fat from skin laxity and muscle banding, and your swallowing history, medications and bleeding tendency, previous neck or chin surgery, thyroid or salivary gland findings, cold-related blood disorders and weight stability are reviewed before either approach is recommended.

Concerns this treatment may help

Under-chin fat reduction may be considered for:

  • A persistent double chin that has not responded to weight loss or exercise
  • A soft, undefined transition between chin and neck in profile
  • Submental fullness in a slim person, where the pocket is disproportionate to overall build
  • A jawline blurred by fat rather than by descent of tissue
  • Profile balance, where the concern is most obvious in photographs from the side

What it will not address: loose or crepey neck skin, the vertical cords caused by the platysma muscle, a recessed chin, or generalised weight.

People sometimes book fat reduction for a problem that is actually skin laxity, and are understandably disappointed. Distinguishing the two by examination takes minutes and saves a wasted treatment.

Check Whether Fat Is Really What You Are Seeing →

Who may benefit

Good candidacy usually means: a discrete pocket of fat under the chin, reasonable skin quality that will retract as volume reduces, a stable weight, and expectations set to gradual improvement rather than a transformed jawline.

Candidacy is more complicated when:

  • Skin laxity is a significant part of the picture — reducing volume beneath loose skin can leave it looser
  • Weight is still changing substantially
  • The fullness sits deep to the platysma muscle rather than above it
  • There is a swallowing difficulty, a thyroid or salivary gland issue, or previous neck surgery or treatment that has altered the anatomy

The two routes suit different people. The injectable allows the treated area to be shaped precisely but asks more of you in recovery. The cooling device asks very little in recovery but treats the area the applicator can draw in, and carries a rare complication that does not resolve on its own. Both trade-offs are discussed openly at assessment.

Your consultation

We examine, not just look. Distinguishing fat from skin laxity and from muscle banding is a hands-on assessment, and it decides whether treatment is worth doing at all.

We ask about swallowing. Any history of difficulty swallowing matters a great deal for the injectable option specifically, and we ask directly.

We review your history. Medications including blood thinners, bleeding tendency, previous neck or chin surgery, thyroid conditions, hernia in the treatment area, and any cold-related blood disorders — the last of these being relevant to the cooling option.

We talk about your calendar. Injectable swelling can be genuinely conspicuous. If you have a wedding in three weeks, that changes the plan or the timing, and it is better raised now.

We explain both routes and recommend one, with the reasoning, rather than leaving you to choose between two names.

None of that assessment is billed to you. The consultation is complimentary, so establishing whether the fullness is even fat costs nothing in consultation fees. A booking fee is charged to secure the appointment, which holds the time, covers the physician's time and reduces no-shows.

What treatment is like

If you have the injectable. The area is marked with a treatment grid and numbed with topical anaesthetic and often ice. One of our physicians — Dr. Gidon Frame, Dr. Sarina Govindasamy or Dr. Teri Fisher — places a series of small injections across the fat pad in a single visit. Expect stinging or burning during and shortly after. Swelling begins quickly — often within hours — and is the part patients most consistently underestimate. More than one session is normally needed, spaced apart to let the area settle.

If you have the cooling device. This session is delivered by our experienced laser technicians and estheticians, working directly under the guidance of our physicians. An applicator is placed under the chin and draws the tissue in with suction. The first minutes are intensely cold and the area then goes numb for the remainder. Afterwards the tissue is massaged, which is briefly uncomfortable. You can generally go straight back to your day.

Either way, this is not a one-appointment fix. Fat clearance is gradual, and a course is usual.

Results and treatment planning

What under-chin fat reduction can improve: the volume of the fat pad beneath the chin, and with it the sharpness of the profile and the chin-to-neck transition.

What it cannot do: tighten loose skin, soften vertical neck bands, project a small chin, treat fat elsewhere, or replace weight management.

When change appears. Nothing is visible immediately — and with the injectable, early swelling makes the area look fuller before it looks better. Both approaches work by having the body clear affected fat cells, which takes weeks to months. Improvement accumulates across a course rather than appearing after one session.

Series and maintenance. More than one session is usual for both routes, spaced at intervals your physician sets according to how the area responds. Once the fullness has been reduced to a satisfactory point, ongoing maintenance sessions are not routinely required.

What influences the outcome: how much fat is present, where it sits relative to the platysma muscle, skin quality and how well it retracts, your age, and weight stability afterwards.

Variation is real. Fat pads differ in size, depth and how readily the body clears treated cells, so two people having the same number of sessions can end up with visibly different degrees of change.

Recovery and aftercare

After the injectable. Swelling under the chin is expected and can be marked, sometimes for a week or two, along with bruising, tenderness, numbness and firm or lumpy areas as things settle. Many people plan a few quieter days. Numbness can persist after swelling has gone.

After the cooling device. Redness, swelling, tenderness, tingling, cramping and numbness are typical and generally milder. Numbness in particular can last for weeks. Most people return to normal activity the same day.

What helps, either way: sleeping with your head elevated, cool compresses if advised, loose collars, and patience while numbness resolves.

Contact the clinic promptly if you notice:

  • An uneven smile, or weakness at one corner of the mouth
  • Any difficulty swallowing
  • Skin that breaks down, ulcerates or looks discoloured at a treated site
  • Fever, spreading redness, or worsening rather than improving pain
  • A firm area that grows over the months after cooling treatment, rather than shrinking

Common side effects

These differ by approach, so they are listed separately.

After injectable treatment, commonly:

  • Swelling, which can be pronounced and visible to others
  • Bruising
  • Pain, stinging or burning at the treated area
  • Numbness, which may persist for weeks
  • Areas of firmness or lumpiness under the skin
  • Redness and a sensation of tightness

After cooling-device treatment, commonly:

  • Redness, firmness and mild swelling
  • Tenderness, aching or cramping
  • Tingling, stinging or altered sensation
  • Numbness in the treated area, often lasting some weeks
  • Transient blanching or skin sensitivity

Less common but important risks

The two approaches carry genuinely different risks. Neither list substitutes for the other.

Risks specific to the injectable:

  • Marginal mandibular nerve injury — irritation or injury to the nerve supplying the lower lip, causing an uneven smile or weakness at one corner of the mouth. This is typically temporary but can take weeks to months to resolve.
  • Difficulty swallowing — reported after treatment and requiring prompt medical attention.
  • Injection-site ulceration or tissue damage, particularly if the medication is placed outside the intended fat plane.
  • Prolonged numbness of the chin or neck.
  • Swelling that is more marked or longer-lasting than anticipated.
  • Infection at an injection site, hair loss at the treated area, or an uneven result requiring further treatment.

Risks specific to the cooling device:

  • Paradoxical adipose hyperplasia — an uncommon reaction in which the treated fat enlarges and firms instead of reducing, typically appearing months afterwards. It does not resolve on its own and correction generally requires surgery.
  • Prolonged numbness or altered sensation persisting well beyond the usual few weeks.
  • Late-onset pain beginning days after the session, which can be significant and may need medical management.
  • Skin changes including bruising, blistering, or changes in pigment.
  • Contour irregularity, or an under-corrected result where the applicator could not capture all of the fat.

When treatment may need to wait

For either approach, treatment may need to be deferred or avoided with:

  • Active infection, inflamed skin or an open lesion in the treatment area
  • Pregnancy or breastfeeding
  • Previous surgery, liposuction or other treatment that has altered the anatomy of the area
  • A neck lump, thyroid or salivary gland abnormality, or enlarged lymph nodes that have not been investigated
  • Weight that is still changing substantially
  • A medical condition affecting healing, or immunosuppression

Particular to the injectable: any history of difficulty swallowing, bleeding disorders or anticoagulant therapy, and known sensitivity to the medication.

Particular to the cooling device: cold-related blood disorders including cryoglobulinaemia, cold agglutinin disease and paroxysmal cold haemoglobinuria, and conditions aggravated by cold or by suction on the area.

This is not a complete screening list. Your medical history, medications, previous procedures and the examination findings are reviewed before either treatment is offered.

Where skincare fits

Honestly, not much here. No product applied to the skin reduces the fat pad beneath the chin, and any that claims to is overpromising.

Where skincare does matter is the skin covering the area. If neck skin quality is part of what bothers you — crepeyness, texture, sun damage — that is a separate concern with separate treatments, and reducing the fat underneath will not fix it. Daily sun protection on the neck is worth doing regardless.

FAQ

Questions about Under-Chin Fat Reduction.

By examination, and it is the first thing we check. Fat under the chin feels like a discrete pocket that can be gently gathered; laxity behaves differently when the skin is moved, and muscle bands become visible when you tense the neck. Many people have some combination. Getting this right determines whether fat reduction will help you at all.

The cooling device, clearly. Most people return to normal activity the same day with redness and numbness as the main after-effects. Injectable treatment produces swelling that can be conspicuous for a week or two, which is worth planning around. Recovery is often the deciding factor for people with fixed social or work commitments.

The nerve that controls the lower lip runs close to the treated area, and irritation or injury to it can weaken one corner of the mouth, giving an asymmetric smile. It is uncommon, usually temporary, and can take weeks to months to settle. Report it to us promptly if it happens so it can be documented and followed.

It is an uncommon reaction to cooling treatment in which the treated fat becomes larger and firmer rather than smaller, typically showing up several months later. Unlike most side effects it does not resolve by itself, and correcting it generally means surgery. It is rare, but you should know it exists before choosing the device route.

Fat cells cleared from the treated area do not regenerate there. However, the fat cells that remain can still enlarge, so significant weight gain can restore fullness under the chin. Keeping your weight reasonably stable is what protects the result.

Sometimes. The area can also be considered alongside treatments aimed at skin quality or laxity, since fat reduction and skin tightening address different problems. Sequencing matters, and treating an area that is still swollen or numb from a previous session is not sensible, so any combination is planned rather than improvised.

The injectable stings or burns during the session and the area is often sore afterwards, though topical anaesthetic and ice help. The cooling device is intensely cold for the first minutes before numbness sets in, and the post-treatment massage is briefly uncomfortable. A minority of people develop significant pain days after cooling treatment, which we can help manage.

Longer than most people expect — weeks rather than days is common after either approach, and it often outlasts the swelling. It is usually described as an odd, muffled sensation rather than something painful, and it resolves gradually. Numbness that persists well beyond that should be reviewed.

It depends which of the two routes you have, and the difference comes down to whether a needle is involved. The injectable is performed by one of our physicians — Dr. Gidon Frame, Dr. Sarina Govindasamy or Dr. Teri Fisher — because placement relative to the nerves under the chin is what keeps the treatment safe. The cooling device uses no needles, and those sessions are delivered by our experienced laser technicians and estheticians, working directly under the guidance of those same physicians. Your assessment, and the decision about which route suits you, is made by a physician either way.

That appointment is complimentary as a consultation — there is no fee for the examination or for the discussion of injectable versus cooling. A booking fee is charged to hold the slot in the diary, which accounts for the physician's time and helps reduce no-shows.
