Men's sexual-health treatment in Vancouver

Understanding Men’s Sexual-Health Treatment.

"Penile enhancement," sometimes searched as "penis enlargement," covers a few different physician-led treatments that address different things. Some men want more girth, some want the penis to look longer at rest, some are concerned about the appearance of the glans, and some are concerned about sensitivity or ejaculatory control. These are separate treatments with separate aims, and the right one, if any, depends on what is actually bothering you.

At Anti-Aging Medical & Laser Clinic in Vancouver, Dr. Gidon Frame and Dr. Teri Fisher assess your concern honestly and explain the options, their limits and their risks, so you can make an informed decision without pressure to proceed.

Treatment options and technologies

In-clinic treatment is one of several clinically recognised approaches to reduced erectile firmness and related concerns, and it is not always the appropriate one — established medical treatment, treating a contributing cardiovascular, metabolic or hormonal factor, or specialist referral is frequently the better route.

The options available here, selected according to clinical findings:

  • Alma Duo — low-intensity acoustic-wave therapy, considered in a short needle-free series where reduced blood flow is a contributing factor. The evidence for this use is emerging rather than settled, and it is not a replacement for first-line care.
  • Platelet-Rich Plasma (PRP) — prepared from your own blood and injected in clinic, considered in selected cases. Its use for sexual health is investigational, and outcomes are uncertain and vary.

These are considered alongside established medical treatment rather than in place of it, and no result is guaranteed.

The selection is a clinical decision, not one you are expected to make. Your symptoms and how long they have been present are reviewed, examination is performed where relevant, and your cardiovascular and metabolic health, hormonal status, medications, alcohol, smoking, sleep, stress and relationship factors are assessed — with blood tests covering hormones, blood sugar or lipids where indicated — before any option is offered.

Concerns this treatment may help

Girth enhancement (injectable filler).

Filler placed beneath the skin of the shaft to temporarily add thickness or circumference. It adds girth only — not erect length — and is not a treatment for erectile dysfunction. Learn about penile girth enhancement →

Lengthening (botulinum toxin).

Botulinum toxin injected near the base to relax the muscles that retract the penis, so it tends to hang lower and look longer when flaccid. It changes flaccid appearance only, not erect length, and is an off-label, emerging use. Learn about penile lengthening →

Glans filler

Premature ejaculation and cosmetic glans enhancement. Hyaluronic-acid filler placed in the head of the penis, used for two different purposes: reducing glans sensitivity as part of a premature-ejaculation program, or cosmetic glans enhancement for the size, shape or appearance of the glans. These are separate goals, assessed separately, and both are covered on our premature ejaculation and glans page. Learn more →

Each option explains what the treatment can and cannot do, the evidence behind it, and the risks. These are cosmetic or emerging treatments; several involve using a product outside its approved (licensed) indication, which your physician will explain.

What penile enhancement does not cover

These treatments are about appearance, girth and, in the case of the glans options, cosmetic shape or ejaculatory control. They are not treatments for erectile dysfunction, and they do not address low testosterone. If your concern is getting or keeping an erection, that is assessed separately through our Men's Sexual-Health Treatment. Lower energy, libido, strength or mood can relate to low testosterone, commonly known as andropause, which we assess and treat as its own service.

Assessment comes first

Whatever option you are considering, it starts with an honest assessment of your concern, your anatomy and your expectations. Normal penile size and shape vary widely, and being within that normal range does not mean you need treatment. If worries about size are causing significant distress, further assessment or psychological support may be recommended first — that is part of caring for your wellbeing, not dismissing your concern.[1]

Consultation planning

Step one is always assessment by one of our physicians, including treating any contributing medical factor that has been identified.

Acoustic-wave sessions. Short in-clinic appointments, typically given as a series, delivered by our experienced laser technicians and estheticians working directly under the guidance of our physicians. A handpiece is applied to the area and delivers pressure waves. There are no needles. Most men describe a tapping or pulsing sensation rather than pain, and go straight back to their day.

Platelet-rich plasma. Because it is a needle-based procedure, it is performed by a physician — Dr. Gidon Frame or Dr. Teri Fisher. Blood is drawn from your arm, prepared, and injected into the treatment area. Local anaesthetic is used for comfort.

How many sessions and how far apart depends on the option and on your assessment, and is agreed with you rather than fixed before you arrive.

Results and what to expect

What treatment may improve. Erectile firmness and reliability for some men, particularly where reduced blood flow is a contributing factor. Sensitivity concerns are sometimes helped. Managing an underlying medical driver can improve function alongside your general health.

What it cannot do. None of these options is a guaranteed cure. They will not resolve difficulty that is primarily psychological or relational, they will not fix a hormonal deficiency that needs treating in its own right, and they will not undo the effect of an untreated cardiovascular or metabolic condition. Nothing here is a substitute for established medical treatment where that is indicated.

When change might appear. Where energy-based or regenerative options do help, the effect builds over weeks rather than arriving after a single visit. Improvements from treating an underlying medical factor follow the timeline of that treatment.

Series and maintenance. Emerging options are typically given as a course, and any benefit is not assumed to be permanent. Repeat treatment is sometimes considered.

What influences the outcome: the underlying cause, your vascular and metabolic health, diabetes, smoking, medications, how long the concern has been present, and psychological factors.

Response varies widely between individuals, and with emerging treatments more than with established ones. Some men notice a clear difference; some notice little.

Afterwards

Acoustic-wave therapy. Usually no downtime. Some men notice temporary sensitivity or a mild ache in the treated area that settles quickly.

After injections. Temporary tenderness, swelling or bruising at the injection sites is common and generally settles over a few days.

Practical advice: keep the area clean, avoid vigorous activity or sexual activity for the period your physician specifies, and do not apply heat or ice unless you have been told to.

Contact the clinic promptly if you develop increasing pain, spreading redness, fever, difficulty passing urine, or a lump or firm area that is not settling.

Possible side effects of the in-clinic options

These apply to the in-clinic procedures only, not to assessment or to management of an underlying medical condition.

After acoustic-wave therapy:

  • Temporary tenderness or sensitivity in the treated area
  • Transient redness
  • A mild ache for a short period afterwards

After platelet-rich plasma injections:

  • Tenderness, swelling or bruising at the injection sites
  • Temporary firmness or lumpiness under the skin
  • Bruising at the site where blood was drawn
  • Short-lived discomfort as the local anaesthetic wears off

Most of these settle within a few days.

Less common but important risks

Uncommon effects of the in-clinic options can include:

  • Infection at an injection site
  • Significant bruising or bleeding, more likely on anticoagulant medication or with a bleeding disorder
  • Persistent pain, nodules or firm areas in the treated tissue
  • Altered sensation in the area
  • Scarring at an injection site
  • A vasovagal episode — feeling faint — during a blood draw or injection
  • No improvement at all, which is a genuine possibility with an emerging treatment

The most consequential risk is not procedural. It is treating the symptom while an undiagnosed cardiovascular, metabolic or hormonal condition goes unaddressed. That is the reason assessment comes first.

When treatment may need to wait

An in-clinic option may be deferred or avoided if you have:

  • An active genital, skin or urinary-tract infection
  • An unexplained lump, lesion or skin change in the treatment area — this needs diagnosing first
  • A bleeding disorder, or you are taking anticoagulant or antiplatelet medication
  • A penile implant or other device in the area
  • Peyronie's disease or significant scarring, which needs assessment in its own right
  • Untreated cardiovascular disease, uncontrolled diabetes or an untreated hormonal condition
  • An active blood or platelet disorder, or a current infection affecting the blood, where platelet-rich plasma is being considered
  • Cancer under active treatment in or near the area
  • Persistent or painful erections, or any acute symptom needing urgent care

This is not a complete screening list. Your full medical history, medications, previous treatments and examination findings are reviewed before anything is offered.

  • Acoustic-wave sessions delivered by experienced laser technicians and estheticians, working directly under the guidance of those same physicians.

FAQ

Questions about Men’s Sexual-Health Treatment.

An examination is often part of a thorough assessment, but you will be told what is proposed and why before anything happens, and you can decline any part of it. Some of the most useful information comes from your history and blood tests rather than from examination.

No. The consultation is complimentary, so the conversation and assessment cost you nothing. A booking fee is taken to secure the appointment time with the physician, which keeps the schedule reliable and reduces missed appointments. The team explains it when the appointment is arranged.

The blood vessels supplying the penis are smaller than those supplying the heart, so vascular disease can show up there earlier. For a meaningful number of men, erectile difficulty is the first noticeable symptom of a cardiovascular problem. Assessing it is not defensive medicine; it is the point.

Your assessment, and any injection-based treatment such as platelet-rich plasma, is carried out by one of our physicians — Dr. Gidon Frame or Dr. Teri Fisher. Anything involving a needle stays with them. Acoustic-wave sessions use no needles, and those are delivered by our experienced laser technicians and estheticians, working directly under the guidance of the same physicians. You will know before your appointment which of the two you are booked for.

Most men describe it as a tapping or pulsing sensation rather than pain, and no anaesthetic is usually needed. Tell the technician treating you if it becomes uncomfortable — delivery can be adjusted.

Anticoagulant and antiplatelet medication significantly raises the risk of bruising and bleeding with any injection-based treatment, so platelet-rich plasma may not be appropriate. Bring a full list of your medications, including anything bought over the counter, and never stop a prescribed blood thinner on your own.

Probably not, and that is worth knowing before you spend anything. Performance anxiety, stress, depression and relationship strain are common contributors and respond to different help entirely. The assessment is designed to distinguish these, and psychological and physical causes frequently coexist.

These are private treatments and are generally not publicly funded. Ask the clinic for current fees at the time of enquiry, and be sceptical of any provider unwilling to be specific about cost before you start a course.

Not on your own initiative. Established treatments are first-line for good reason, and emerging in-clinic options are generally considered alongside them rather than instead of them. Any change to prescribed medication should be a decision made with the physician who prescribed it.

Yes, if you want to. Sexual difficulty affects both people in a relationship, and some men find the conversation easier with their partner present. It is entirely your choice, and part of the appointment can be conducted alone if you prefer.

That is a real possibility with an emerging option, and you should go in knowing it. If a course produces no benefit, the sensible next step is to review the assessment rather than repeat the same treatment, and to look again at established options or specialist referral.
